Work Forms
Upon accepting a job, the student brings a copy of the offer letter to his or her co-op advisor. The advisor then plots out the student’s projected School-Work Alternation Schedule (Adobe PDF file) . Students are asked to give a copy of the alternation schedule to their supervisor at the beginning of the first work term.
The advisor will also discuss the work forms that the student is responsible for submitting to the Division of Professional Practice each term. The forms include an Assignment Record (MS Word | Adobe PDF) that is completed with the supervisor during the first two weeks of each work term and a Student Performance Evaluation Form (MS Word | Adobe PDF) that is completed by the supervisor during the last two weeks of each work term. We encourage supervisors to schedule a performance review meeting to discuss the completed evaluation with the student before submitting a copy to our office.
